Site Browsing:

• Major Topics are listed at left.
• Sub Topics appear in Menus at top and bottom of scroll area.
• GO TO links are underscored.  "Click" on link to go, "Click" the "Back" button at screen top to return.

To print a form:

• Select the Forms option from the left hand menu.
• When the Forms option page appears, select a form from the identical items listed at both the top and bottom of the screen.
• When the form appears in the form window, move your cursor to the very upper left hand corner of the form "white-space".  Press down the left mouse button and drag the cursor into the form.  The entire form should be highlighted at this point. 
• Select the FILE option from the browser menu at the top of the page.
• From the "pull-down" menu, select PRINT.
• When the PRINT specification page appears, select to print just the SELECTION.
• Press OK to print.
